Vitamin D testing is covered only when medically necessary, focusing on individuals with specific signs, symptoms, or risk factors for deficiency.
Claims for vitamin D tests are reviewed at submission, and coverage depends on the diagnosis provided. Only diagnoses that meet the criteria outlined in medical policy 2.04.507 Testing Serum Vitamin D Levels are eligible. Before proceeding with testing, consider whether the individual meets these criteria to ensure appropriate and effective use of resources.
