Update to 2024 Premera Amazon Plans

Amazon discontinued the in network only (INO) plans with Premera as of January 1, 2024. This was the only Amazon plan with Premera that had a primary care physician (PCP) referral requirement to see a specialist. Therefore, we removed the Amazon referral form from our provider website.

We continue to offer other Premera plans to Amazon members in 2024, but those plans don’t include PCP and referral requirements.
