Submitting Appeals

To increase efficiency and provide the best possible service, we’d like to remind you of an important requirement for submitting appeals.
Please submit one appeal document per patient. Don’t combine multiple patient appeals into a single document.
Why is this important?
When appeals for multiple patients are combined into a single document, it can delay processing and increase the risk of errors. Separating appeals by patient helps us with the following:
1. Ensure accuracy
2. Process appeals faster
3. Comply with privacy regulations
