Submit Appeals Using the Correct Forms, Not Through Availity

We’ve had some cases where providers are submitting appeals through the Availity Claim Attachment tool. That tool is only available for claims that are either in process or finalized and require additional information for further processing.
To get your appeal processed as quickly as possible, please use the correct forms, and submit by fax or mail and not through Availity. These forms can be found on our provider website forms page. Be sure to bookmark this page for the latest forms and instructions.
Online appeals feature coming soon
While appeals currently can’t be submitted through Availity, know that we’ll be adding this feature in the fall.
