Premera Blue Cross HMO Referral Update

Effective January 25, 2024, most HMO-approved referrals are extended to 24 visits for 12 months. This is a change from 6 visits in 6 months. This helps decrease the administrative burden for providers and improve the member experience. Please continue to submit referrals to Premera Blue Cross HMO through Availity for the fastest turnaround, or submit referrals by fax at 888-704-2091.
