Member Outreach to Encourage Preventive Care

This month, select members with specific care gaps will receive communications informing them of the importance of the preventive screening visit and urge them to make an appointment. There will be two different types of outreach efforts:

  1. Members due for one of the following will receive a letter:
  2. Members due for more than one of the following cancer screenings or chronic condition testing will receive a personalized scorecard.

If members have not received a screening or test, more information about it is included. If members have received a screening or test, a congratulations message is included. Here’s a sample checklist:
