Entering Diabetic Retinal Eye Exam Results into Medical Records
Premera strives to ensure diabetic patients get the care they need. We encourage members with diabetes to have their diabetic retinal eye exam (DRE) every 1-2 years or as recommended by their eye specialist. To make sure the results of these important exams are documented in the patient’s medical record for review and follow-up by their primary care provider (PCP), we created a form that providers can give their patients to take with them to their eye exam.
The form can either be faxed back to the PCP’s office by the eye care provider or the patient can bring the document to their PCP at their next appointment. The form is also available in Spanish.
