At-Home Preventive Health Kits

Eligible Premera Blue Cross members can get free at‑home tests, including a colorectal cancer fecal immunochemical test (FIT) kit (ages 45–75) and a diabetes A1c test. This makes it easier for your patients to complete preventive tests without coming into the office.

Why it Matters

Home test kits remove common barriers like scheduling, travel, and time off work. Patients can test at home, and you still get the results.

How it Works

There’s no need for you to enroll patients. We invite eligible members by letter, email, or text.

• Patients collect samples at home using the kit.
• Kits are returned using prepaid packaging.
• Results are sent to both the patient and you.
• We send reminders if the kit isn’t returned.
