Some eligible Premera Blue Cross members with type 2 diabetes may receive reminders to complete their annual A1c test. These reminders may include information about the HbA1c home test kit program, which allows members to complete the test from home.

• Let your patients know the home test kit is a convenient option for completing their annual A1c test.
• Remind patients that the home test kit does not replace their regular diabetes care and follow-up visits.
• Encourage patients to complete and return the kit as soon as possible.
• Continue discussing diabetes management, medications, lab results, and follow-up care during visits.
• Document completed A1c tests and diabetes-related care in the patient’s medical record.
